Before taking Xeljanz (tofacitinib), tell your healthcare provider about your complete medical history, including any infections, heart conditions, immune system disorders, allergies, or history of cancer.
Pregnancy and Breastfeeding
Xeljanz should not be used during pregnancy unless specifically recommended by your healthcare provider. It may harm an unborn baby.
It is not known whether tofacitinib passes into breast milk. Because of the potential risk to a nursing infant, breastfeeding is generally not recommended while taking Xeljanz. Talk to your healthcare provider if you are pregnant, planning to become pregnant, breastfeeding, or planning to breastfeed.
Serious Infections
Xeljanz affects the immune system and may increase the risk of serious infections, including infections that can become life-threatening.
Your risk may be higher if you:
- Have a weakened immune system
- Have a history of recurrent infections
- Take other medications that suppress the immune system
Contact your healthcare provider immediately if you develop symptoms of an infection, such as:
- Fever or chills
- Persistent cough
- Shortness of breath
- Unexplained fatigue
- Painful skin sores or wounds
Avoid close contact with people who have contagious illnesses, such as chickenpox, measles, or influenza.
Cardiovascular Risk
Xeljanz carries a boxed warning for an increased risk of major cardiovascular events, including:
- Heart attack
- Stroke
- Cardiovascular death
Tell your healthcare provider if you have heart disease or risk factors such as:
- High blood pressure
- High cholesterol
- Diabetes
- Smoking history

Cancer Risk
Xeljanz may increase the risk of certain cancers in some patients.

Allergic Reactions
Do not take Xeljanz if you are allergic to tofacitinib or any of the ingredients in the medication.
