Lovenox (active ingredient: enoxaparin sodium) is a prescription medication used to help prevent and treat blood clots. It belongs to a class of medications called low molecular weight heparins (LMWHs). These medications are also called anticoagulants, or blood thinners.

Although they are often called blood thinners, medications like Lovenox do not actually make the blood thinner. Instead, they reduce the blood’s ability to form harmful clots.

Thromboembolic Disorders

Lovenox is used for thromboembolic disorders, which are conditions where blood clots form inside blood vessels and interfere with normal blood flow. These clots can become dangerous if they grow larger, block circulation, or travel to another part of the body.

Lovenox may be prescribed to help prevent or treat:

  • Deep vein thrombosis (DVT), which is a blood clot that usually forms in a deep vein of the leg
  • Pulmonary embolism (PE), which happens when a blood clot travels to the lungs and blocks blood flow
  • Blood clots after certain surgeries, such as hip replacement or knee replacement surgery
  • Blood clots in people who are hospitalized or have limited movement due to illness
  • Certain heart-related conditions, such as unstable angina or acute coronary syndrome

Blood Clot Prevention After Surgery

After major surgery, especially hip or knee replacement surgery, the risk of blood clots may increase because movement is limited during recovery. When the legs are not moving as much, blood flow can slow down, making clots more likely to form.

Lovenox may be prescribed after surgery to lower this risk and help prevent clots from forming in the legs or traveling to the lungs.

Blood Clot Treatment

Lovenox may also be used to treat existing blood clots, such as DVT or PE. In this case, it helps prevent the clot from getting larger while the body gradually breaks it down over time.

Depending on the condition being treated, Lovenox may be used alone or along with another anticoagulant medication, such as warfarin.

Heart-Related Conditions

Lovenox may be used in certain heart conditions where blood clots can worsen reduced blood flow to the heart. These conditions may include unstable angina and certain types of heart attack or acute coronary syndrome.

By reducing clot formation, Lovenox can help lower the risk of further blockage in the blood vessels that supply the heart.

How Lovenox Works

Lovenox works by increasing the activity of antithrombin III, a natural protein in the body that helps control blood clotting. This action blocks certain clotting factors, especially factor Xa, which plays an important role in forming blood clots.

By slowing down the clotting process, Lovenox helps reduce the risk of harmful clots while still allowing the body to maintain normal clotting functions needed to stop bleeding from injuries.

Important Note

Because Lovenox affects blood clotting, it can increase the risk of bleeding. It should be used exactly as prescribed, and your healthcare provider may monitor you depending on your condition, dose, kidney function, and other medications.

Lovenox comes as a prefilled syringe. It is usually given as an injection under the skin, also called a subcutaneous injection. In some cases, it may be given intravenously (IV) in a hospital setting.

Lovenox should not be injected into a muscle. Injecting it into the muscle can increase the risk of bleeding or bruising.

How Often Lovenox Is Used

Lovenox is commonly given once or twice daily, depending on the condition being treated and your healthcare provider’s instructions. Treatment may last for several days or longer. In some cases, treatment may last about 10 to 14 days, but the exact length of treatment depends on your medical condition and risk of blood clots.

Use Lovenox exactly as prescribed. Do not inject more or less than directed, and do not use it more often than your healthcare provider recommends.

Starting Lovenox in the Hospital

Lovenox is often started in the hospital and may be continued at home. If you need to use it at home, your healthcare provider will show you how to give the injection safely.

Carefully follow the directions on your prescription label. If you are unsure how to inject Lovenox or have questions about your dose, ask your healthcare provider or pharmacist before using it.

Recommended Dosage

The recommended dose of Lovenox depends on several factors, including:

  • The condition being treated
  • Your body weight
  • Your age
  • Your kidney function
  • Your risk of bleeding
  • Whether it is being used to prevent or treat a blood clot

For prevention of deep vein thrombosis (DVT), a common dose is 40 mg once daily by subcutaneous injection.

For prevention of blood clots after certain surgeries, a common dose may be 30 mg twice daily or 40 mg once daily, given by subcutaneous injection.

Your healthcare provider will determine the dose and treatment length that are right for you. Always follow their instructions exactly.

Before using Lovenox, tell your healthcare provider about your medical history, especially if you have a bleeding disorder, uncontrolled high blood pressure, low platelet count, stomach ulcer, diabetic eye disease, kidney disease, or a history of stroke or recent surgery.

Bleeding Risk

Lovenox is an anticoagulant, which means it reduces the blood’s ability to form clots. Because of this, it can increase the risk of bleeding. In some cases, bleeding can be serious or even life-threatening.

Use Lovenox with caution if you have a higher risk of bleeding, including if you have:

  • A history of bleeding problems
  • Uncontrolled high blood pressure
  • Low platelet count
  • Stomach or intestinal ulcers
  • Eye problems caused by diabetes
  • Recent surgery or injury

Contact your healthcare provider right away if you notice unusual bleeding, easy bruising, blood in your urine or stool, black or tarry stools, vomiting blood, severe headache, dizziness, weakness, or bleeding that does not stop.

Low Platelets From Heparin

Tell your healthcare provider if you have ever taken heparin and developed a low platelet count, a condition known as heparin-induced thrombocytopenia (HIT). This is important because Lovenox is related to heparin and may not be appropriate for some people with this history.

Spinal or Epidural Hematoma Warning

Lovenox carries a boxed warning for the risk of spinal or epidural hematomas. These are serious blood collections that can form around the spine, especially in people receiving spinal anesthesia, epidural anesthesia, or spinal puncture while taking anticoagulants.

This complication is rare but serious and may lead to long-term or permanent paralysis. The risk may be higher if you have an epidural catheter, a history of spinal surgery or repeated spinal procedures, or if you take other medications that affect bleeding, such as NSAIDs, aspirin, or blood thinners.

Seek urgent medical attention if you develop symptoms such as back pain, numbness, tingling, muscle weakness, or loss of bladder or bowel control.

Follow Dosing Instructions Carefully

Use Lovenox exactly as prescribed. Do not change your dose, skip doses, or stop treatment unless your healthcare provider tells you to do so. Carefully following your dosing instructions helps reduce the risk of both bleeding and blood clots during treatment.

How should enoxaparin be stored?

Enoxaparin should be stored at room temperature, away from excess heat, light, and moisture. Do not freeze it. Keep it in its original packaging until you are ready to use it. Any unused or expired medication, needles, or syringes should be discarded safely according to your healthcare provider’s or pharmacist’s instructions.

Can enoxaparin be used during pregnancy?

Enoxaparin may be used during pregnancy when a healthcare provider determines that the benefits outweigh the possible risks. Pregnancy can increase the risk of blood clots in some people, so anticoagulant treatment may be necessary. If you are pregnant, planning to become pregnant, or breastfeeding, discuss the risks and benefits with your healthcare provider before using enoxaparin.

How long does it take for enoxaparin to start working?

Enoxaparin starts working fairly quickly after it is injected, usually within a few hours. However, the length of treatment and the overall effect depend on the condition being treated, your dose, and your individual risk of blood clots or bleeding.

Can enoxaparin be taken with food?

Enoxaparin is not taken by mouth. It is given as an injection, usually under the skin. Because it is injected, food does not affect how it is taken. You can eat normally unless your healthcare provider gives you specific dietary instructions.

Are there any drug interactions with enoxaparin?

Yes. Enoxaparin can interact with medications that increase the risk of bleeding. These may include NSAIDs, such as ibuprofen or naproxen; aspirin; antiplatelet medications; other blood thinners; and certain other drugs that affect blood clotting. Tell your healthcare provider about all prescription medications, over-the-counter medicines, vitamins, and supplements you are taking before starting enoxaparin.
