Emsam Patch contains selegiline, a prescription antidepressant used to treat major depressive disorder (MDD) in adults. It is a transdermal patch, which means the medicine is absorbed through the skin and delivered gradually into the bloodstream over 24 hours.

How Does Emsam Work?

Emsam belongs to a group of medicines called monoamine oxidase inhibitors, or MAOIs.

It works by blocking monoamine oxidase, an enzyme that breaks down certain chemical messengers in the brain. This increases the availability of chemicals such as serotonin, norepinephrine, and dopamine, which are involved in mood regulation.

By changing the activity of these brain chemicals, Emsam can help improve symptoms of depression.

What Symptoms Can Emsam Help With?

Emsam is used to treat the overall symptoms of major depressive disorder. Depending on the person, treatment may help improve:

  • Low mood
  • Loss of interest or pleasure
  • Low energy
  • Sleep problems
  • Difficulty concentrating
  • Changes in appetite
  • Other symptoms related to depression

Improvement may happen gradually, and the full antidepressant effect can take time.

How is Emsam Different From Antidepressant Tablets?

Emsam is applied to the skin instead of being swallowed.

The patch slowly releases selegiline through the skin throughout the day. A new patch is applied once every 24 hours to clean, dry, intact skin on the upper torso, upper thigh, or outer upper arm.

What Strengths Are Available?

Emsam is available in three strengths:

  • 6 mg per 24 hours
  • 9 mg per 24 hours
  • 12 mg per 24 hours

The usual starting and target dose is 6 mg per 24 hours. If needed, a healthcare provider may increase the dose by 3 mg per 24 hours at intervals of at least 2 weeks, up to a maximum of 12 mg per 24 hours.

Why Are Food Restrictions Important With Emsam?

MAOIs can interact with tyramine, a natural substance found in certain aged, fermented, cured, or spoiled foods.

Too much tyramine while taking an MAOI can cause a sudden and dangerous increase in blood pressure called a hypertensive crisis.

For the 6 mg per 24-hour patch, a special low-tyramine diet is generally not required. However, people using the 9 mg or 12 mg patches must avoid tyramine-rich foods and beverages.

These dietary restrictions should continue for 2 weeks after stopping the 9 mg or 12 mg patch or after reducing the dose to 6 mg.

Why Are Medication Interactions Important?

Emsam can have serious interactions with certain medicines, including some antidepressants, opioid pain medicines, cough medicines containing dextromethorphan, and other drugs that affect serotonin or blood pressure.

Because some interactions can be dangerous, tell your healthcare provider about all prescription medicines, over-the-counter products, vitamins, supplements, and herbal products before starting Emsam.

Do not start or stop another medication while using Emsam without first checking with your healthcare provider.

How Long Is Emsam Used?

Treatment length varies from person to person.

Major depressive episodes may require several months or longer of continued treatment. If Emsam is used for an extended period, your healthcare provider should periodically review whether it is still helping and whether continued treatment is appropriate.

Important Reminder

Emsam is a once-daily selegiline patch used to treat major depressive disorder in adults. It works by increasing the activity of brain chemicals involved in mood regulation.

The usual starting dose is 6 mg per 24 hours, but higher strengths may be prescribed when needed. Because Emsam is an MAOI, food and medication interactions are particularly important, especially with the 9 mg and 12 mg patches.

Emsam contains selegiline and is worn on the skin for 24 hours at a time. A new patch is applied once every day, preferably at about the same time each day. Wear only one patch at a time.

Where Should You Apply Emsam?

Apply Emsam to clean, dry, intact skin on one of these areas:

  • Upper torso, below the neck and above the waist
  • Upper thigh
  • Outer upper arm

Choose a different application site each day. Do not use the same spot two days in a row, since rotating the site can help reduce skin irritation.

Prepare the Skin Before Applying the Patch

Wash the chosen area with soap and warm water, then dry it completely.

Do not apply the patch to skin that is oily, hairy, irritated, broken, scarred, or calloused. Avoid putting lotion, oil, powder, or other skin products on the area before applying Emsam.

How to Apply the Patch

Remove the Emsam patch from its sealed pouch. Peel away part of the protective liner without touching too much of the sticky surface.

Place the sticky side onto your skin, remove the remaining liner, and press the patch firmly into place with your fingers.

Make sure the entire patch is securely attached to your skin. Do not place it where tight clothing may constantly rub against it and cause it to come loose.

Remove the Old Patch First

Before applying your next patch, remove the old one.

You should never routinely wear more than one Emsam patch at the same time unless your healthcare provider specifically instructs you otherwise. Each patch is designed to release a particular amount of selegiline over 24 hours.

Recommended Starting Dose

The usual starting and target dose is Emsam 6 mg per 24 hours, applied as one patch every 24 hours.

Your healthcare provider will decide whether this dose provides enough benefit before considering an increase.

Increasing the Dose

If additional treatment is needed, your healthcare provider may increase the dose in steps of 3 mg per 24 hours.

Available strengths are:

  • 6 mg per 24 hours
  • 9 mg per 24 hours
  • 12 mg per 24 hours

Dose increases should generally be made at intervals of at least 2 weeks. The maximum recommended dose is 12 mg per 24 hours.

Do not use extra patches or switch to a stronger patch on your own.

How Long Does Emsam Take to Work?

The full antidepressant effect may be delayed, so you may not notice the complete benefit immediately after starting treatment.

Continue using Emsam exactly as prescribed unless your healthcare provider tells you to change or stop treatment.

Food Restrictions With Higher Doses

Special food restrictions become important with the 9 mg and 12 mg per 24-hour patches.

At these strengths, you need to avoid foods and beverages that contain large amounts of tyramine, because combining high tyramine intake with an MAOI can cause a dangerous increase in blood pressure.

These restrictions begin on the first day you use the 9 mg or 12 mg patch and should continue for 2 weeks after stopping those strengths or reducing your dose to 6 mg per 24 hours.

What About the 6 mg Patch?

A special low-tyramine diet is generally not required with the 6 mg per 24-hour strength when used as directed.

However, medication interactions are still very important with every Emsam strength because selegiline is an MAOI.

Use in Older Adults

For adults 65 years of age and older, the recommended Emsam dose is 6 mg per 24 hours.

A healthcare provider should determine whether Emsam is appropriate based on other medical conditions and medications.

Important Reminder

Apply one Emsam patch every 24 hours to clean, dry, healthy skin. Remove the old patch before applying a new one and rotate application sites each day.

The usual starting dose is 6 mg per 24 hours. If needed, a healthcare provider may gradually increase the dose to 9 mg and then 12 mg per 24 hours, with at least 2 weeks between increases. Higher strengths require important dietary restrictions, so follow your healthcare provider’s instructions carefully.

Suicidal Thoughts or Changes in Behavior

Like other antidepressants, Emsam may increase the risk of suicidal thoughts or behavior in some younger people, especially when treatment is first started or when the dose is changed.

Contact your healthcare provider promptly if you notice new or worsening depression, unusual behavior changes, agitation, or thoughts of self-harm.

Serotonin Syndrome

Emsam can cause a potentially life-threatening condition called serotonin syndrome if it is combined with certain medicines that increase serotonin.

Symptoms may include agitation, confusion, sweating, fever, fast heartbeat, muscle stiffness, shaking, poor coordination, nausea, vomiting, or diarrhea. Seek immediate medical care if these symptoms occur.

Dangerously High Blood Pressure

Emsam can sometimes cause a sudden and severe increase in blood pressure, known as a hypertensive crisis.

The risk is especially important with the 9 mg and 12 mg patches if you eat foods high in tyramine. These strengths require dietary restrictions.

Seek immediate medical care if you develop a severe headache, stiff neck, racing heartbeat, chest pain, or other sudden unusual symptoms.

Food Restrictions

If you use Emsam 9 mg or 12 mg per 24 hours, avoid foods and drinks that contain large amounts of tyramine.

A special low-tyramine diet is generally not required with the 6 mg patch, but medication interactions are still important at every dose.

Serious Medication Interactions

Do not combine Emsam with certain medicines unless your healthcare provider has carefully managed the required waiting period.

Important examples include SSRIs, SNRIs, clomipramine, imipramine, tramadol, meperidine, methadone, pentazocine, dextromethorphan, and some other serotonergic medicines. These combinations can cause serious reactions such as serotonin syndrome.

Tell Your Healthcare Provider About All Medicines

Before using Emsam, tell your healthcare provider about all prescription medicines, over-the-counter products, vitamins, supplements, and herbal products you take.

Cold medicines and weight-loss products containing stimulants such as pseudoephedrine or phenylephrine may raise blood pressure when combined with Emsam.

Dizziness and Fainting

Emsam may cause dizziness, lightheadedness, or fainting, especially when standing up quickly.

Rise slowly from sitting or lying down, and use caution until you know how the medication affects you.

Driving and Alertness

Emsam may affect your ability to think clearly, react quickly, or stay alert.

Use caution when driving, operating machinery, or doing other activities that require concentration until you know how Emsam affects you.

Alcohol and Other Substances

Talk to your healthcare provider before drinking alcohol while using Emsam.

Alcohol or other substances that affect the nervous system may worsen dizziness, drowsiness, or other side effects.

Medical Conditions

Tell your healthcare provider about all medical conditions before starting Emsam, especially heart or blood pressure problems, seizures, liver problems, or kidney problems.

This helps your healthcare provider determine whether Emsam is appropriate and whether additional monitoring is needed.

Pregnancy and Breastfeeding

If you are pregnant, planning to become pregnant, or breastfeeding, talk to your healthcare provider before using Emsam.

Your healthcare provider can review the potential benefits and risks and decide whether treatment is appropriate.

Skin Irritation

Emsam can cause irritation where the patch is applied. You may notice redness, itching, swelling, or discomfort at the application site.

Rotate the patch location each day to help reduce irritation. Contact your healthcare provider if the reaction becomes severe or does not improve.

Avoid Heat on the Patch

Do not expose the Emsam patch to direct external heat such as heating pads, electric blankets, heat lamps, saunas, hot tubs, heated water beds, or prolonged direct sunlight.

Heat may increase how much selegiline is absorbed through your skin.

Important Reminder

Emsam is an MAOI and can have serious food and medication interactions. Always check with your healthcare provider before starting or stopping another medicine while using Emsam, including over-the-counter cough and cold products.

What Foods Should I Avoid While Using Emsam Patch?

Food restrictions depend on the strength of Emsam you are using. If you use the 6 mg per 24-hour patch, a special low-tyramine diet is generally not required. However, if you use the 9 mg or 12 mg per 24-hour patch, you should avoid foods and drinks that contain large amounts of tyramine. High-tyramine foods can include certain aged cheeses, cured or fermented meats, fermented foods, and some alcoholic beverages. Eating large amounts of tyramine while using the higher-strength patches can cause a sudden and dangerous increase in blood pressure. Continue following the low-tyramine diet for 2 weeks after stopping the 9 mg or 12 mg patch or after reducing the dose to 6 mg.

Can I Use Emsam Patch During Pregnancy or Breastfeeding?

If you are pregnant or planning to become pregnant, talk to your healthcare provider before using Emsam. The medication may pose risks to the developing baby, so your healthcare provider should carefully consider the potential benefits and risks. Breastfeeding is not recommended while using Emsam and for 5 days after the final patch because of the possibility of serious side effects in a breastfed baby.

What Should I Do If I Miss an Emsam Patch?

If you forget to apply a new Emsam patch, apply one when you remember and then continue according to the schedule recommended by your healthcare provider. Do not wear two patches at the same time to make up for a missed application. If you are unsure how to return to your regular schedule after a missed patch, contact your healthcare provider or pharmacist.

Can Emsam Patch Be Used With Other Medications?

Emsam can have serious or even life-threatening interactions with certain medications. Some antidepressants and other medicines that increase serotonin should not be combined with Emsam because they can increase the risk of serotonin syndrome. Certain opioid pain medicines, including meperidine, and some cough, cold, decongestant, or stimulant products can also cause dangerous interactions. Before starting Emsam, tell your healthcare provider about all prescription medicines, over-the-counter products, vitamins, herbal supplements, and recreational substances you use. Do not start, stop, or change another medication while using Emsam without checking with your healthcare provider first. Some medicines also require a specific waiting period before Emsam can be started or after Emsam is stopped.
