Take Relpax as soon as possible after your migraine headache begins, following your healthcare provider’s instructions. It should not be used for ordinary tension headaches or for headaches that have not been diagnosed as migraines.

Relpax is available as a tablet that you take by mouth to treat a migraine attack after it begins. It is not taken every day to prevent migraines.

When should you take Relpax?

Take Relpax as soon as possible after your migraine headache begins. It can be used for a migraine with or without aura.

You do not need to take Relpax during the aura phase unless your healthcare provider has specifically instructed you to do so. Follow the directions on your prescription label.

Relpax may be taken with or without food. Swallow the tablet with water.

What is the usual dose?

The usual starting dose for adults is either 20 mg or 40 mg. Your healthcare provider will select the dose based on your medical history, migraine severity, response to treatment, and possible side effects.

The maximum amount that should be taken as one dose is 40 mg. A higher single dose has not been shown to provide enough additional benefit to outweigh the increased risk of side effects.

When can you take a second dose?

If your migraine improves after the first dose but later returns, you may take a second dose at least 2 hours after the first one.

A second dose may also be taken if your migraine has not completely resolved after 2 hours, provided your healthcare provider has told you that it is appropriate.

Do not take more than 80 mg within any 24-hour period. This means you should not take more than two 40 mg tablets in one day.

What if the first dose does not help?

If the first dose provides no relief, contact your healthcare provider before taking Relpax for another migraine attack. Your healthcare provider may need to confirm that your headaches are migraines or recommend another treatment.

Do not repeatedly take extra doses when the first dose has not worked unless your healthcare provider has given you clear instructions.

How quickly does Relpax work?

Relpax is designed to treat an active migraine attack. Some people may notice an improvement within a few hours, but the response varies.

Relief may include reduced headache pain, nausea, and sensitivity to light or sound. Relpax does not work for every migraine attack, and symptoms may sometimes return after initially improving.

How often can Relpax be used?

Relpax should only be taken when needed for a migraine attack. It should not be taken every day to prevent migraines.

The safety of treating an average of more than three migraine attacks within a 30-day period has not been established. Tell your healthcare provider if you need Relpax frequently or if your migraines are becoming more common.

Medication-overuse headaches

Using Relpax or other acute headache medications too frequently may lead to medication-overuse headaches. This can cause headaches to occur more often, last longer, or become more difficult to treat.

Tell your healthcare provider if you use migraine medication on 10 or more days each month. They may need to review your treatment and consider a preventive migraine medication.

Do not combine Relpax with another triptan or an ergot-containing migraine medication unless your healthcare provider has confirmed that the timing is safe.

  • When each migraine begins

When should you seek medical attention?

Seek immediate medical help if your headache is sudden and extremely severe, feels different from your usual migraines, or occurs with fainting, confusion, fever, neck stiffness, seizures, difficulty speaking, sudden vision loss, or weakness on one side of your body.

You should also seek urgent care if you experience chest pain, severe chest pressure, shortness of breath, an irregular heartbeat, or severe abdominal pain after taking Relpax.

Take Relpax exactly as prescribed. Do not increase your dose, take doses less than 2 hours apart, or exceed 80 mg within 24 hours.

Who should not take Relpax?

Relpax may not be safe for people with certain heart, blood vessel, or neurological conditions. Do not take it if you have:

  • Coronary artery disease, angina, or a previous heart attack
  • Coronary artery spasms, including Prinzmetal angina
  • A history of stroke or transient ischemic attack
  • Peripheral artery disease
  • Uncontrolled high blood pressure
  • Certain serious heart rhythm disorders
  • Hemiplegic or brainstem-aura migraine
  • Severe liver impairment
  • An allergy to eletriptan or any ingredient in Relpax

Kidney disease is not automatically a reason to avoid Relpax. However, blood pressure increases have been observed in people with kidney impairment, so your healthcare provider may need to monitor you more closely.

Medication interactions

Tell your healthcare provider and pharmacist about every prescription medication, over-the-counter product, vitamin, and supplement you use.

Do not take Relpax within 24 hours of:

Another triptan, such as sumatriptan, rizatriptan, or zolmitriptan

An ergot-containing migraine medication, such as ergotamine or dihydroergotamine

Using these medicines too close together may cause excessive narrowing of the blood vessels.

Relpax must also not be used within at least 72 hours of a strong CYP3A4 inhibitor. These medications can greatly increase the amount of eletriptan in your bloodstream. Examples include ketoconazole, itraconazole, clarithromycin, ritonavir, nelfinavir, and nefazodone.

Antidepressants and serotonin syndrome

Relpax is not automatically prohibited with selective serotonin reuptake inhibitors, serotonin-norepinephrine reuptake inhibitors, tricyclic antidepressants, or monoamine oxidase inhibitors. However, combining these medications can increase the risk of serotonin syndrome.

Seek immediate medical attention if you develop agitation, confusion, hallucinations, fever, heavy sweating, a fast heartbeat, unstable blood pressure, shaking, stiff muscles, poor coordination, nausea, vomiting, or diarrhea.

Pregnancy and breastfeeding

Tell your healthcare provider if you are pregnant, planning to become pregnant, or breastfeeding. Available information is not sufficient to clearly determine the pregnancy risks of eletriptan, so your healthcare provider will weigh the expected benefits against the possible risks.

Eletriptan passes into breast milk. Exposure to a nursing baby may be reduced by avoiding breastfeeding for 24 hours after taking Relpax. Discuss the safest feeding plan with your healthcare provider.

Dizziness and drowsiness

Relpax may cause dizziness, weakness, or drowsiness. Do not drive, operate machinery, or perform activities that require full alertness until you know how the medication affects you.

Using Relpax correctly

Take Relpax only after a migraine attack begins. It is used to treat migraines with or without aura, but it does not prevent future attacks.

Use it only for headaches that your healthcare provider has diagnosed as migraines. Seek medical attention if a headache is sudden, extremely severe, or different from your usual migraines.

Do not exceed the prescribed dose. The maximum recommended amount is 80 mg within 24 hours, with at least 2 hours between doses. Taking more than directed may increase the risk of high blood pressure, heart problems, blood vessel spasms, and other serious side effects.

Grapefruit

The official Relpax prescribing information does not specifically require patients to avoid grapefruit or grapefruit juice. However, eletriptan is processed by the CYP3A4 enzyme, and strong medications that block this enzyme can cause dangerous increases in eletriptan levels.

Ask your healthcare provider or pharmacist whether grapefruit is appropriate for you, particularly if you take other medications that interact with grapefruit.
