Foradil Inhaler (formoterol fumarate) is a prescription bronchodilator used for the long-term management of certain respiratory conditions, including chronic obstructive pulmonary disease (COPD) and asthma.

Foradil contains formoterol fumarate, an active ingredient belonging to a class of medications called long-acting beta2-adrenergic agonists (LABAs).

LABAs work by relaxing the muscles surrounding the airways, allowing air to move more freely into and out of the lungs.

Foradil is available as a dry powder inhalation capsule containing 12 micrograms of formoterol fumarate. The capsule is used with a specially designed Aerolizer inhalation device.

The medication helps prevent airway narrowing and relieve ongoing breathing difficulties associated with certain respiratory conditions.

Foradil is primarily used as maintenance treatment, meaning it helps manage symptoms over time rather than providing emergency relief during a sudden breathing attack.

Important: When used for asthma, Foradil must be prescribed alongside an inhaled corticosteroid. It should never be used alone to treat asthma because LABA treatment without an inhaled corticosteroid increases the risk of serious asthma-related complications.

Treatment of Chronic Obstructive Pulmonary Disease (COPD)

Foradil is used for the long-term maintenance treatment of chronic obstructive pulmonary disease (COPD).

COPD is a long-term lung condition that causes persistent breathing difficulties due to airflow obstruction.

The condition commonly includes chronic bronchitis, emphysema, or a combination of both.

In people with COPD, the airways may become inflamed and narrowed, while damage to the air sacs can make it harder to move air out of the lungs.

These changes can cause shortness of breath, wheezing, chest tightness, and a persistent cough.

Foradil helps manage COPD by relaxing the muscles surrounding the airways, making breathing easier.

The medication may help improve the following symptoms:

  • Shortness of Breath: Foradil helps widen narrowed airways, allowing more air to move through the lungs and reducing breathing discomfort.
  • Wheezing: When airways become narrow, breathing may produce a whistling sound. Foradil helps relax airway muscles, which may reduce wheezing.
  • Chest Tightness: Narrowed airways can create a feeling of pressure or tightness in the chest. By improving airflow, Foradil may help relieve this discomfort.
  • Breathing Difficulties During Activity: People with COPD may become breathless while walking, climbing stairs, or performing everyday activities. Regular treatment may help improve breathing during physical activity.
  • Nighttime Breathing Symptoms: Foradil provides long-lasting bronchodilation that may help reduce symptoms occurring during the night.
  • Airflow Limitation: By relaxing the muscles around the bronchial tubes, Foradil helps improve the movement of air through the respiratory system.

Foradil does not cure COPD or reverse permanent lung damage.

Instead, it helps control symptoms and maintain more open airways during ongoing treatment.

The medication may be used alongside other COPD treatments, such as long-acting muscarinic antagonists (LAMAs) or inhaled corticosteroids, when appropriate.

Your healthcare provider will determine which combination of medications is most suitable for your symptoms and medical history.

Treatment of Asthma

Foradil may also be prescribed as an additional maintenance treatment for asthma when symptoms are not adequately controlled with an inhaled corticosteroid.

Asthma is a chronic inflammatory condition that affects the airways inside the lungs.

People with asthma have sensitive airways that may react to allergens, exercise, respiratory infections, cold air, or other triggers.

During an asthma episode, the muscles surrounding the airways tighten, while inflammation and mucus production may further restrict airflow.

This can cause wheezing, coughing, chest tightness, and shortness of breath.

Foradil helps prevent airway narrowing by relaxing the smooth muscles around the bronchial tubes.

It may help manage the following asthma symptoms:

  • Wheezing: Helps keep airways open, reducing the whistling sound associated with narrowed bronchial tubes.
  • Shortness of Breath: Improves airflow, making it easier to breathe during normal daily activities.
  • Chest Tightness: Helps reduce the muscle tightening that contributes to breathing discomfort.
  • Nighttime Asthma Symptoms: Long-lasting bronchodilation may help prevent certain breathing symptoms that interrupt sleep.
  • Recurring Bronchospasm: Helps prevent repeated tightening of airway muscles when used according to the prescribed maintenance schedule.

However, Foradil does not adequately treat the underlying airway inflammation responsible for asthma.

Foradil must always be used with an inhaled corticosteroid when prescribed for asthma.

Inhaled corticosteroids help reduce airway inflammation, while formoterol helps relax airway muscles.

This combination addresses different aspects of asthma management.

Using Foradil alone for asthma may increase the risk of serious asthma-related complications, including hospitalization and death.

Foradil should not replace your prescribed inhaled corticosteroid or rescue medication.

Prevention of Exercise-Induced Bronchospasm

Foradil has also been used to prevent exercise-induced bronchospasm in certain patients.

Exercise-induced bronchospasm occurs when physical activity causes the muscles surrounding the airways to tighten.

This may lead to coughing, wheezing, chest tightness, or shortness of breath during or after exercise.

Foradil helps relax airway muscles before physical activity, reducing the likelihood of bronchospasm.

In the U.S. product information, Foradil Aerolizer was indicated for occasional prevention of exercise-induced bronchospasm in adults and children aged five years and older.

For this purpose, the medication was administered at least 15 minutes before exercise.

However, people already taking Foradil twice daily should not use additional doses before exercising.

Patients with asthma must also continue appropriate inhaled corticosteroid treatment.

The approved indications and age restrictions vary by country, and current asthma guidelines often favor other treatment approaches.

Your healthcare provider will determine whether a formoterol-containing medication is suitable for preventing exercise-related breathing symptoms.

How Does Foradil Inhaler Work?

Foradil contains formoterol fumarate, a long-acting beta2-adrenergic receptor agonist.

Formoterol works by stimulating beta2 receptors found on the smooth muscles surrounding the bronchial tubes.

These receptors help regulate whether the airway muscles tighten or relax.

When formoterol activates beta2 receptors, it triggers a series of chemical reactions that cause the airway muscles to relax.

This process is called bronchodilation.

As the muscles relax, the airways widen, allowing air to move more easily into and out of the lungs.

Activates Beta2-Adrenergic Receptors

Beta2-adrenergic receptors are located on smooth muscle cells surrounding the airways.

When formoterol binds to these receptors, it activates an enzyme called adenylate cyclase.

This enzyme increases the production of cyclic adenosine monophosphate (cAMP), a chemical messenger inside cells.

Higher cAMP levels help relax airway smooth muscles.

As these muscles relax, the bronchial tubes widen, improving airflow.

Relaxes Airway Smooth Muscles

During bronchospasm, the muscles surrounding the airways contract, making the passages narrower.

This can cause wheezing, chest tightness, and difficulty breathing.

Foradil reduces this muscle contraction by activating beta2 receptors.

As the muscles relax, the airways become wider and air can pass through more easily.

This helps reduce the breathing difficulties associated with COPD and asthma.

Provides Long-Lasting Bronchodilation

Foradil is classified as a long-acting bronchodilator because its effects can last approximately 12 hours.

Formoterol has a relatively rapid onset of bronchodilator activity compared with some other long-acting beta2 agonists.

However, Foradil Aerolizer is not intended to treat sudden breathing attacks, even though its active ingredient begins working relatively quickly.

The medication is prescribed according to a maintenance schedule to help control symptoms over time.

Your healthcare provider should prescribe an appropriate rescue medication for sudden breathing difficulties.

Helps Maintain Airflow Throughout the Day

People with COPD and asthma may experience recurring airway narrowing that interferes with normal breathing.

Foradil helps maintain airway relaxation between scheduled doses.

This may reduce the frequency of certain respiratory symptoms and improve comfort during daily activities.

For long-term maintenance treatment, the medication is commonly administered every 12 hours.

Consistent use may help maintain bronchodilation, but it does not replace other treatments needed to manage underlying airway inflammation or prevent disease progression.

How Long Does Foradil Take to Work?

Foradil begins relaxing airway muscles relatively soon after inhalation.

Bronchodilation may begin within several minutes, although the time needed to notice symptom improvement varies between individuals.

The medication’s effects can last for approximately 12 hours.

For this reason, Foradil is generally prescribed twice daily for maintenance treatment of COPD or asthma.

Although Foradil may begin widening the airways quickly, it should not be used as an emergency rescue treatment for sudden shortness of breath or an acute asthma attack.

If you experience sudden breathing difficulties, use the rescue medication prescribed by your healthcare provider.

Seek emergency medical attention if severe breathing problems do not improve or you have difficulty speaking because of breathlessness.

Important Information About Foradil Inhaler

Foradil is supplied as dry powder capsules that must be inhaled using the Aerolizer device.

Do not swallow Foradil capsules. They are designed for inhalation only and should not be taken by mouth.

Each capsule contains a measured dose of formoterol fumarate.

Foradil should not be used more frequently than prescribed because excessive doses may cause serious side effects, including a rapid heartbeat, tremors, palpitations, and abnormal heart rhythms.

Tell your healthcare provider if you have heart disease, high blood pressure, diabetes, thyroid problems, or a history of seizures.

Do not combine Foradil with another long-acting beta2 agonist unless your healthcare provider has specifically reviewed your treatment.

If you are using Foradil for asthma, continue your prescribed inhaled corticosteroid. Never use Foradil alone for asthma control.

Foradil helps manage breathing difficulties associated with COPD and selected asthma treatment plans by relaxing airway muscles and improving airflow. Its long-lasting bronchodilator effects help maintain more open airways, but it does not cure these conditions or replace emergency rescue treatment.

Foradil Inhaler is a prescription long-acting bronchodilator used to manage breathing difficulties associated with chronic obstructive pulmonary disease (COPD) and, in certain treatment plans, asthma.

Foradil contains formoterol fumarate, a medication belonging to a class of drugs called long-acting beta2-adrenergic agonists (LABAs). It works by relaxing the muscles surrounding the airways, helping improve airflow and making breathing easier.

Foradil comes as a dry powder capsule containing 12 micrograms (mcg) of formoterol fumarate. The capsule must be placed inside the specially designed Aerolizer inhalation device.

Do not swallow Foradil capsules. The medication must be inhaled through the mouth using the Aerolizer device.

For maintenance treatment, the usual adult dosage is one capsule inhaled twice daily, once in the morning and once in the evening, approximately 12 hours apart.

Your healthcare provider will determine the appropriate dosage and treatment schedule based on your condition and response to treatment.

How to Use the Foradil Aerolizer

Follow these steps to use the Foradil Aerolizer correctly:

  1. Wash and dry your hands: Clean your hands thoroughly with soap and water. Make sure they are completely dry before handling the capsules, as moisture can affect the dry powder medication.
  2. Prepare the inhaler: Remove the protective cap from the Aerolizer device and open the capsule compartment according to the manufacturer’s instructions.
  3. Remove one capsule from the packaging: Take a Foradil capsule out of its foil blister immediately before use. Do not remove capsules in advance because exposure to moisture may affect the medication.
  4. Place the capsule inside the inhaler: Insert the capsule into the capsule-shaped chamber at the base of the Aerolizer. Do not place the capsule directly into the mouthpiece.
  5. Close the inhaler: Return the mouthpiece to its closed position until it clicks securely into place.
  6. Pierce the capsule: Hold the Aerolizer upright and press both side buttons inward once to puncture the capsule. Release the buttons completely before inhaling.
  7. Breathe out away from the inhaler: Exhale fully, but do not breathe into the mouthpiece. Moisture from your breath may cause the powder to clump.
  8. Position the mouthpiece: Place the mouthpiece between your lips and close your lips around it to create a secure seal.
  9. Inhale quickly and deeply: Breathe in rapidly and deeply through your mouth. You may hear a whirring sound as the capsule spins and releases the powder.
  10. Hold your breath: Remove the inhaler from your mouth and hold your breath for as long as is comfortable, usually several seconds.
  11. Breathe out slowly: Exhale gently away from the inhaler.
  12. Check the capsule: Open the Aerolizer and inspect the capsule. If powder remains, close the inhaler and repeat the inhalation steps until the powder has been inhaled. Do not press the piercing buttons again.
  13. Remove the empty capsule: Once the medication has been inhaled, discard the empty capsule safely.
  14. Close the inhaler: Replace the protective cap and store the device in a clean, dry location.

Always use the Aerolizer device supplied for Foradil. Do not place the capsules into another inhaler or use the Aerolizer with a spacer.

Recommended Dosage of Foradil

The usual dosage depends on the respiratory condition being treated.

Condition General dosage
COPD maintenance treatment One 12 mcg capsule inhaled twice daily, approximately 12 hours apart
Asthma maintenance treatment One 12 mcg capsule twice daily, only with an inhaled corticosteroid and when prescribed
Prevention of exercise-induced bronchospasm One 12 mcg capsule inhaled at least 15 minutes before exercise, when prescribed

Do not take more than two 12 mcg capsules in a 24-hour period unless your healthcare provider has provided instructions consistent with your specific product’s approved dosing.

Foradil must not be used alone for asthma. It must be combined with an inhaled corticosteroid to manage the underlying airway inflammation.

Foradil is not intended for emergency relief of sudden asthma attacks or severe breathing difficulties.

Important Tips When Using Foradil

Use a New Aerolizer With Each Refill

Always use the Aerolizer inhalation device supplied with your Foradil prescription or refill.

The Aerolizer is designed to pierce the capsule and release the dry powder during inhalation.

Do not use a spacer or another type of inhaler with Foradil capsules.

Replace your old Aerolizer with the new device provided with your refill.

Keep the inhaler dry and clean. Do not wash it with water, as moisture may interfere with the dry powder medication.

Keep Capsules Sealed Until Use

Store Foradil capsules in their original foil blister packaging until you are ready to use them.

The packaging helps protect the medication from moisture.

Remove only one capsule at a time, immediately before your scheduled dose.

Make sure your hands are completely dry before handling the capsule.

Do not swallow, chew, crush, or open the capsules. They must be used with the Aerolizer inhaler.

Do Not Exhale Into the Inhaler

Before inhaling Foradil, breathe out fully while keeping your mouth away from the inhaler.

Exhaling into the mouthpiece may introduce moisture into the device, causing the powder to clump.

This can interfere with medication delivery.

After breathing out, place the mouthpiece between your lips and inhale quickly and deeply.

You should hear a whirring sound as the capsule spins inside the inhaler.

If you do not hear the sound, check that the capsule can spin freely. Do not press the piercing buttons again.

Check That the Capsule Is Empty

After inhaling, open the Aerolizer and inspect the capsule.

If some powder remains, close the inhaler and repeat the inhalation process.

Most people can empty the capsule with one or two inhalations.

Do not pierce the capsule a second time because it may break into small pieces.

Once the capsule is empty, remove it from the inhaler and discard it.

Use Foradil Before Exercise Only When Prescribed

Foradil may be prescribed to prevent exercise-induced bronchospasm, a condition that causes airway narrowing during or after physical activity.

When prescribed for this purpose, the usual dose is one 12 mcg capsule inhaled at least 15 minutes before exercise.

Do not use another dose of formoterol for at least 12 hours.

If you already use Foradil twice daily for maintenance treatment, do not take an additional capsule before exercise.

If you have asthma, continue using your prescribed inhaled corticosteroid. Foradil should not be used alone to manage persistent asthma.

Contact your healthcare provider if exercise frequently causes wheezing, coughing, or shortness of breath despite treatment.

Using Foradil With Other Inhalers

If you use other inhaled medications, follow the treatment schedule recommended by your healthcare provider.

Some people with COPD or asthma require more than one inhaler to manage their symptoms.

Your healthcare provider may prescribe an inhaled corticosteroid, another maintenance medication, or a rescue inhaler for sudden breathing difficulties.

When taking different inhaled medications, follow the spacing instructions for each product. Your healthcare provider may recommend waiting approximately one minute between certain inhalations, although this is not a universal requirement.

Do not combine Foradil with another long-acting beta2 agonist unless your healthcare provider has specifically reviewed your medications.

What Should I Do If I Miss a Dose?

If you forget to take a scheduled dose of Foradil, skip the missed dose and take your next dose at the usual time.

Do not take the missed dose later or take two capsules together to make up for it.

Taking extra formoterol may increase the risk of serious side effects, including a rapid heartbeat, palpitations, tremors, and abnormal heart rhythms.

Try to use Foradil at approximately the same times each day, with morning and evening doses about 12 hours apart.

If you frequently miss doses or are unsure about your treatment schedule, contact your healthcare provider or pharmacist.

How Long Does Foradil Take to Work?

Foradil begins relaxing the muscles surrounding your airways within several minutes after inhalation.

The medication helps widen the bronchial tubes, allowing air to move more easily into and out of your lungs.

Its bronchodilator effects can last approximately 12 hours.

This is why Foradil is usually prescribed twice daily for long-term maintenance treatment.

However, Foradil Aerolizer is not intended to provide emergency relief during sudden asthma attacks or acute breathing difficulties.

If you experience sudden shortness of breath, use the rescue inhaler prescribed by your healthcare provider.

If your rescue medication does not provide adequate relief or your breathing becomes severely difficult, seek emergency medical attention.

When to Contact Your Healthcare Provider

Contact your healthcare provider if Foradil no longer controls your usual breathing symptoms or if you experience increasing wheezing, coughing, chest tightness, or shortness of breath.

You should also report persistent tremors, nervousness, dizziness, or an unusually fast heartbeat during treatment.

Stop using Foradil and seek immediate medical attention if your breathing suddenly becomes worse immediately after inhalation. This may indicate paradoxical bronchospasm, a rare but serious reaction in which the airways unexpectedly narrow.

Seek emergency medical attention if you develop severe breathing difficulties, chest pain, fainting, or swelling of your face or throat.

Always use Foradil exactly as prescribed. Proper Aerolizer technique, consistent dosing, careful capsule handling, and avoiding additional doses can help maintain airflow while reducing medication-related risks.

Before using Foradil Inhaler, tell your healthcare provider about any medical conditions, allergies, or medications you are taking.

Foradil is a prescription long-acting bronchodilator used to manage certain breathing conditions, including chronic obstructive pulmonary disease (COPD) and asthma when prescribed alongside an inhaled corticosteroid.

Foradil contains formoterol fumarate, a medication belonging to a class of drugs called long-acting beta2-adrenergic agonists (LABAs).

It works by relaxing the muscles surrounding the airways, helping keep them open and making breathing easier.

Although Foradil can improve airflow, it may cause side effects involving the heart, nervous system, blood sugar, and potassium levels.

People with cardiovascular disease, diabetes, seizure disorders, or thyroid problems may require additional precautions.

Foradil Aerolizer is not a rescue inhaler and must never be used alone to treat asthma. Follow your prescribed treatment schedule and contact your healthcare provider if your breathing symptoms become worse.

Asthma Treatment and Inhaled Corticosteroids

Foradil should only be prescribed for asthma when used alongside an inhaled corticosteroid.

Asthma is a chronic inflammatory condition that causes the airways to become swollen, sensitive, and narrowed.

Foradil helps relax the muscles surrounding the bronchial tubes, allowing air to move more freely through the lungs.

However, Foradil does not adequately treat the underlying airway inflammation responsible for asthma.

Inhaled corticosteroids help reduce this inflammation and lower the risk of serious asthma attacks.

Using a LABA such as Foradil without an inhaled corticosteroid increases the risk of serious asthma-related complications, including hospitalization and death.

If your healthcare provider prescribes Foradil for asthma, continue using your inhaled corticosteroid even when your breathing improves.

Do not stop or reduce your corticosteroid dosage without medical advice.

Foradil is generally considered an additional maintenance treatment when asthma symptoms are not adequately controlled with inhaled corticosteroids alone.

Your healthcare provider should regularly reassess your asthma control and determine whether ongoing LABA treatment is necessary.

Not for Sudden Breathing Problems

Foradil Aerolizer should not be used to treat sudden asthma attacks, acute bronchospasm, or rapidly worsening breathing symptoms.

Although formoterol begins relaxing airway muscles relatively quickly, Foradil Aerolizer is intended for maintenance treatment rather than emergency relief.

Your healthcare provider should prescribe an appropriate rescue inhaler for sudden breathing difficulties.

Signs that your respiratory condition may be worsening include:

  • Increasing Shortness of Breath: You become breathless more frequently or during activities that previously caused little difficulty.
  • Persistent Wheezing: Wheezing becomes more noticeable or does not improve with your prescribed treatment.
  • Chest Tightness: You experience worsening pressure or tightness in your chest.
  • Frequent Rescue Inhaler Use: You need your rescue medication more often than usual.
  • Nighttime Symptoms: Coughing, wheezing, or breathlessness repeatedly interrupts your sleep.

Do not use extra Foradil capsules to relieve sudden symptoms.

If your breathing becomes worse or your usual rescue medication is no longer providing adequate relief, seek immediate medical evaluation.

Seek emergency medical attention if you experience severe breathing difficulties or cannot speak comfortably because of breathlessness.

Increased Risk of Asthma-Related Complications

Foradil should not be started in patients experiencing rapidly worsening or unstable asthma.

LABA medications can cause serious complications when used without appropriate anti-inflammatory treatment.

The risk is particularly concerning when asthma symptoms are poorly controlled or when patients rely on bronchodilators instead of using their prescribed inhaled corticosteroids.

Foradil may make breathing feel easier while underlying airway inflammation remains uncontrolled.

This can delay recognition of worsening asthma if symptoms are not monitored carefully.

Your healthcare provider should review your treatment if you experience increasing wheezing, frequent nighttime symptoms, reduced lung function, or a growing need for rescue medication.

Do not increase your Foradil dosage or use it more frequently than prescribed.

If your asthma remains uncontrolled, your healthcare provider may recommend adjusting your inhaled corticosteroid treatment or changing your maintenance medication.

Cardiovascular Disease and High Blood Pressure

Foradil should be used with caution in people with cardiovascular conditions, including hypertension, coronary artery disease, heart failure, and abnormal heart rhythms.

Formoterol stimulates beta2-adrenergic receptors to relax airway muscles. However, it may also affect the cardiovascular system.

In some individuals, Foradil may increase heart rate, change blood pressure, or cause palpitations.

These effects may be more concerning in people with existing heart conditions.

Possible symptoms include:

  • Rapid Heartbeat: Your heart may beat faster than usual.
  • Palpitations: You may feel that your heart is racing, pounding, or skipping beats.
  • Chest Discomfort: You may experience unusual tightness or pressure.
  • Dizziness: Changes in blood pressure or heart rhythm may cause lightheadedness.
  • Irregular Heartbeat: Your heart rhythm may feel uneven or unpredictable.

Tell your healthcare provider if you have high blood pressure, a history of heart attack, or another cardiovascular condition.

Your healthcare provider may recommend monitoring your blood pressure, heart rate, or heart rhythm during treatment.

Seek emergency medical attention if you develop severe chest pain, fainting, or a persistent irregular heartbeat.

QT Prolongation and Abnormal Heart Rhythms

Foradil may affect the electrical signals that control your heartbeat.

In some individuals, formoterol may contribute to QT interval prolongation, which occurs when the heart takes longer than normal to reset electrically between beats.

A prolonged QT interval can increase the risk of potentially serious heart rhythm problems in susceptible people.

The risk may be higher if you already have a heart rhythm disorder, low potassium levels, or take medications that affect the QT interval.

Tell your healthcare provider if you have a history of irregular heartbeat, unexplained fainting, or congenital long QT syndrome.

Your healthcare provider may recommend an electrocardiogram (ECG) or blood tests when appropriate.

Contact your healthcare provider if you notice frequent palpitations or unusual dizziness.

Seek emergency medical attention if you experience fainting, severe chest discomfort, or significant breathing difficulties associated with an irregular heartbeat.

Paradoxical Bronchospasm

Foradil may rarely cause paradoxical bronchospasm, a serious reaction in which the airways suddenly become narrower immediately after inhaling the medication.

This reaction is called paradoxical because Foradil is normally intended to relax and widen the airways.

Paradoxical bronchospasm can cause sudden breathing difficulties and may become life-threatening.

Possible symptoms include:

  • Sudden Wheezing: Wheezing begins or becomes worse immediately after inhalation.
  • Chest Tightness: You experience unexpected tightening or pressure in your chest.
  • Shortness of Breath: Breathing becomes more difficult after using the medication.
  • Persistent Coughing: You develop sudden coughing or airway irritation.
  • Difficulty Breathing: You may struggle to inhale or exhale normally.

Stop using Foradil immediately if paradoxical bronchospasm occurs.

Use your prescribed fast-acting rescue bronchodilator if available and seek urgent medical attention.

Do not take another dose of Foradil until your healthcare provider has assessed the reaction and recommended an alternative treatment.

Seizure Disorders and Epilepsy

Tell your healthcare provider if you have epilepsy, a history of seizures, or another neurological condition before using Foradil.

Formoterol stimulates beta2-adrenergic receptors and may affect the nervous system in some individuals.

Possible nervous system-related side effects include tremors, nervousness, dizziness, headache, and restlessness.

People with seizure disorders may be more sensitive to certain effects of adrenergic medications.

Your healthcare provider will determine whether Foradil is appropriate based on your seizure history and current medications.

Do not change or discontinue your prescribed seizure medication without medical advice.

If you develop unusual shaking, worsening tremors, or changes in your seizure pattern, contact your healthcare provider.

Seek emergency medical attention for a prolonged seizure or repeated seizures without recovery.

Diabetes and Blood Sugar Changes

Foradil should be used with caution in people with diabetes.

Formoterol may affect blood glucose levels because beta2-adrenergic stimulation can influence how the body releases and uses glucose.

In some people, beta2 agonists may cause blood sugar levels to rise.

These changes are generally uncommon at recommended inhaled doses but may be more concerning in people with poorly controlled diabetes.

Possible symptoms of high blood sugar include:

  • Increased Thirst: Feeling unusually thirsty throughout the day.
  • Frequent Urination: Needing to urinate more often than usual.
  • Unusual Fatigue: Feeling tired or weak.
  • Blurred Vision: Temporary changes in eyesight.
  • Dry Mouth: Persistent dryness or discomfort.

If you have diabetes, your healthcare provider may recommend monitoring your blood glucose more frequently when starting Foradil or changing your dosage.

Tell your healthcare provider if you have a history of diabetic ketoacidosis or difficulty controlling your blood sugar.

Do not change your diabetes medication without consulting your healthcare provider.

Hyperthyroidism and Thyroid Disorders

Tell your healthcare provider if you have hyperthyroidism or another thyroid condition before using Foradil.

Hyperthyroidism occurs when the thyroid gland produces excessive amounts of thyroid hormones.

This can increase your heart rate and make your body more sensitive to certain medications that stimulate adrenergic receptors.

Because Foradil may increase heart rate and cause tremors or nervousness, these effects may become more noticeable in people with an overactive thyroid.

Possible symptoms include:

  • Rapid or pounding heartbeat.
  • Increased sweating.
  • Nervousness or anxiety.
  • Tremors or shaking.
  • Unusual sensitivity to heat.
  • Difficulty sleeping.

Your healthcare provider will assess whether your thyroid condition is adequately controlled before recommending Foradil.

If you experience persistent palpitations, severe tremors, or worsening symptoms, contact your healthcare provider.

Pheochromocytoma

Foradil should be used with caution in people with pheochromocytoma.

Pheochromocytoma is a rare tumour, usually affecting an adrenal gland, that can produce excessive amounts of hormones called catecholamines.

These hormones influence heart rate, blood pressure, and the body’s response to stress.

Because Foradil stimulates adrenergic receptors, it may increase cardiovascular effects in people with this condition.

Possible symptoms of pheochromocytoma include sudden episodes of high blood pressure, headaches, sweating, palpitations, and anxiety.

Tell your healthcare provider if you have been diagnosed with pheochromocytoma or are being evaluated for an adrenal gland disorder.

Your healthcare provider will determine whether another respiratory medication would be more appropriate.

Low Potassium Levels (Hypokalemia)

Foradil may lower potassium levels in the bloodstream.

Potassium is an essential mineral that helps regulate muscle movement, nerve signalling, and normal heart rhythm.

Beta2 agonists such as formoterol can temporarily move potassium from the bloodstream into cells, reducing the amount measured in the blood.

This condition is called hypokalemia.

Low potassium levels may cause:

  • Muscle Weakness: Feeling unusually weak or tired.
  • Muscle Cramps: Painful tightening of muscles.
  • Palpitations: A racing or irregular heartbeat.
  • Fatigue: Reduced energy or difficulty completing normal activities.
  • Severe Heart Rhythm Changes: Significant potassium reductions may contribute to dangerous arrhythmias.

The risk may be higher if you take certain diuretics, corticosteroids, or xanthine medications, such as theophylline.

People with severe respiratory conditions may also be more vulnerable when low oxygen levels are present.

Your healthcare provider may recommend blood tests to monitor potassium levels if you have relevant risk factors.

Do not take potassium supplements unless your healthcare provider recommends them.

Seek medical attention if you experience severe muscle weakness, persistent palpitations, or fainting.

Drug Interactions

Foradil may interact with certain prescription medications, over-the-counter drugs, and other inhaled treatments.

Some interactions may increase the risk of cardiovascular side effects, while others may reduce the effectiveness of formoterol.

Medications that may require additional precautions include:

  • Beta Blockers: Medications such as propranolol and metoprolol may reduce Foradil’s bronchodilator effects. Some beta blockers may also cause airway narrowing in susceptible people.
  • Monoamine Oxidase Inhibitors (MAOIs): Certain medications used to treat depression may increase the cardiovascular effects of formoterol.
  • Tricyclic Antidepressants: Medications such as amitriptyline may increase the risk of heart rhythm changes when used with Foradil.
  • Other Long-Acting Beta2 Agonists: Combining Foradil with medications containing salmeterol, indacaterol, or additional formoterol may increase the risk of overdose and serious cardiovascular reactions.
  • Diuretics: Certain water tablets, including furosemide and hydrochlorothiazide, may increase the risk of low potassium levels.
  • Corticosteroids: Oral or injected corticosteroids may contribute to potassium loss, particularly when used with other medications that lower potassium levels.
  • Theophylline: This medication may increase certain cardiovascular effects and the risk of low potassium.
  • Heart Rhythm Medications: Drugs that prolong the QT interval may increase the likelihood of abnormal heart rhythms.

Not every medication combination will cause a serious interaction.

However, your healthcare provider should review your complete medication list before prescribing Foradil.

Tell your healthcare provider or pharmacist about all prescription drugs, over-the-counter medications, supplements, and inhalers you use.

Do not combine Foradil with another long-acting beta2 agonist unless your healthcare provider has specifically reviewed your treatment plan.

Liver Disease

Tell your healthcare provider if you have liver disease or a history of abnormal liver function before using Foradil.

The liver helps process formoterol after it enters your bloodstream.

People with significant liver impairment may process the medication differently, potentially affecting medication exposure.

However, there is limited information about how severe liver disease affects formoterol levels, and a specific dosage adjustment has not been established for all patients with liver impairment.

Foradil is not generally known to cause significant liver injury when used at recommended inhaled doses.

Your healthcare provider will determine whether additional monitoring is necessary based on your liver condition and other medications.

Contact your healthcare provider if you develop unusual symptoms or have concerns about using Foradil with existing liver disease.

Allergic Reactions and Milk Protein Sensitivity

Do not use Foradil if you are allergic to formoterol fumarate or any other ingredient in the medication.

Foradil dry powder capsules contain lactose, which may contain small amounts of milk proteins.

People with a severe milk protein allergy may experience an allergic reaction to inhaled products containing these proteins.

A milk protein allergy is different from lactose intolerance. People with lactose intolerance have difficulty digesting lactose, while a milk protein allergy involves an immune reaction to proteins found in milk.

Possible symptoms of an allergic reaction include:

  • Skin Rash: Red or itchy patches may appear.
  • Hives: Raised, itchy bumps may develop.
  • Facial Swelling: Your lips, eyelids, tongue, or throat may become swollen.
  • Wheezing: Breathing may become noisy or difficult.
  • Chest Tightness: You may experience pressure or discomfort when breathing.

Tell your healthcare provider if you have a severe milk protein allergy or have reacted to inhaled medications in the past.

Stop using Foradil and seek emergency medical attention if you experience difficulty breathing, swelling of your tongue or throat, or other signs of a severe allergic reaction.

Avoid Excessive Doses

Foradil should only be used at the dosage prescribed by your healthcare provider.

For maintenance treatment, the usual adult dosage is one 12 mcg capsule inhaled twice daily, approximately 12 hours apart.

Taking additional capsules may increase the risk of serious side effects.

Possible symptoms of excessive formoterol exposure include a rapid heartbeat, palpitations, tremors, headache, nervousness, chest discomfort, and abnormal heart rhythms.

Do not use extra Foradil capsules to relieve sudden breathing difficulties.

If your breathing symptoms become worse despite regular treatment, contact your healthcare provider rather than increasing your dosage.

Seek urgent medical advice if you accidentally take more than the prescribed amount, particularly if you develop heart-related symptoms.

Proper Storage of Foradil

Store Foradil capsules according to the instructions supplied with your specific product.

The U.S. Foradil Aerolizer instructions recommend storing the medication at room temperature between 20°C and 25°C (68°F and 77°F) after dispensing.

Keep the capsules in their original foil blister packaging until immediately before use.

The packaging helps protect the dry powder medication from heat and moisture.

Do not remove capsules in advance or transfer them into a pill organizer.

Keep the Aerolizer device dry and use the new inhaler supplied with each refill.

Do not wash the Aerolizer with water, as moisture may affect medication delivery.

Store the medication away from excessive heat, humidity, and direct sunlight.

Keep Foradil out of the reach of children and pets.

Storage requirements can differ between countries and product versions, so always follow the instructions on your packaging.

When to Contact Your Healthcare Provider

Contact your healthcare provider if your breathing symptoms become worse, you need your rescue inhaler more frequently, or Foradil no longer provides the expected symptom control.

You should also report persistent palpitations, tremors, unusual dizziness, muscle cramps, or changes in blood sugar levels.

Stop using Foradil and seek immediate medical attention if you experience sudden worsening of breathing immediately after inhalation.

Seek emergency medical attention immediately if you develop severe breathing difficulties, chest pain, fainting, swelling of your face or throat, or a serious irregular heartbeat.

Foradil is intended for maintenance treatment and should not replace your prescribed emergency rescue medication.

Foradil can help maintain open airways and improve breathing in people with COPD or selected asthma treatment plans. Following the prescribed dosage, continuing inhaled corticosteroids when treating asthma, checking for medication interactions, and recognizing serious side effects can help reduce treatment-related risks.

Is Foradil Inhaler Safe?

Foradil Inhaler can be safe and effective when used as prescribed by a healthcare provider. However, like all medications, it may cause side effects and is not suitable for everyone. Foradil contains formoterol fumarate, a long-acting beta2-adrenergic agonist (LABA) that helps relax the muscles surrounding the airways. This allows air to move more easily into and out of the lungs. Foradil is used for the long-term management of certain respiratory conditions, including chronic obstructive pulmonary disease (COPD) and asthma when prescribed with an inhaled corticosteroid. Foradil must never be used alone to treat asthma. Using a LABA without an inhaled corticosteroid increases the risk of serious asthma-related complications, including hospitalization and death. Foradil is also not intended to provide emergency relief during sudden asthma attacks or severe breathing difficulties. Your healthcare provider should prescribe an appropriate rescue inhaler for these situations. Common side effects may include headache, tremors, nervousness, dizziness, and heart palpitations. More serious reactions, including abnormal heart rhythms and sudden worsening of breathing, may also occur. Tell your healthcare provider if you have heart disease, high blood pressure, diabetes, thyroid problems, or a history of seizures before starting treatment. The age restrictions for Foradil depend on the condition being treated and the regional product information. The historical U.S. prescribing information included certain uses in children aged five years and older, but Foradil is not recommended for children younger than five years. Your healthcare provider will determine whether Foradil is appropriate based on your age, respiratory condition, medical history, and other medications.

What Should I Do If I Miss a Dose of Foradil Inhaler?

If you forget to take a scheduled dose of Foradil, skip the missed dose and take your next dose at the usual time. Foradil is generally prescribed twice daily for maintenance treatment, with doses taken approximately 12 hours apart. Taking your medication at consistent times helps maintain its long-lasting bronchodilator effects and supports ongoing symptom control. Do not take two doses together or inhale extra capsules to make up for a missed dose. Taking more formoterol than prescribed may increase your risk of side effects, including a rapid heartbeat, palpitations, tremors, nervousness, and abnormal heart rhythms. If you frequently miss doses, consider setting a medication reminder or establishing a regular morning and evening inhaler routine. If your breathing symptoms become worse after missing a dose, use your prescribed rescue inhaler if needed. Do not use additional Foradil capsules to treat sudden breathing difficulties. Contact your healthcare provider or pharmacist if you miss several doses or are unsure how to continue your treatment schedule.

Can Foradil Inhaler Be Used Alone, or Should It Be Used With Other Medications?

Whether Foradil can be used alone depends on the respiratory condition being treated. For people with asthma, Foradil must always be used alongside an inhaled corticosteroid. It should never be used as the only medication for asthma control. Asthma causes ongoing inflammation and narrowing of the airways. Foradil helps relax airway muscles, but it does not adequately treat the underlying inflammation. Inhaled corticosteroids reduce inflammation and help lower the risk of serious asthma attacks. Using Foradil alone for asthma may improve breathing temporarily while leaving airway inflammation uncontrolled, increasing the risk of dangerous complications. For people with chronic obstructive pulmonary disease (COPD), Foradil may be prescribed on its own as a long-acting bronchodilator or combined with other maintenance medications. Your healthcare provider may recommend additional inhaled treatments, such as long-acting muscarinic antagonists (LAMAs), depending on the severity of your symptoms and history of COPD flare-ups. However, Foradil should not be combined with another long-acting beta2 agonist unless your healthcare provider has specifically reviewed the treatment plan, as this may increase the risk of excessive medication exposure. Foradil is not a rescue inhaler. If you experience sudden wheezing, chest tightness, or shortness of breath, use the fast-acting rescue medication prescribed by your healthcare provider. Always follow your individualized respiratory treatment plan and do not stop or change your other inhaled medications without medical advice.

How Should I Store Foradil Inhaler?

Foradil Inhaler should be stored at room temperature in a clean, dry place, away from excessive heat and moisture. According to the Foradil Aerolizer prescribing information, the capsules should be stored between 20°C and 25°C (68°F and 77°F) after being dispensed by the pharmacy. Foradil contains formoterol fumarate in dry powder capsules that are used with the specially designed Aerolizer inhalation device. The capsules are sensitive to moisture, which may affect the quality of the powder and how effectively the medication is delivered during inhalation. Keep each capsule in its original foil blister packaging until immediately before use. Do not remove capsules in advance or store them in a pill organizer. Store the Aerolizer in a dry location with its protective cap securely closed. Avoid keeping it in the bathroom, where heat and humidity may change frequently. Do not wash the Aerolizer with water because moisture can interfere with the dry powder medication. Follow the manufacturer’s instructions for cleaning the device. Use the new Aerolizer supplied with each prescription or refill and discard the old device according to the manufacturer’s instructions. Keep Foradil capsules and the inhaler out of the reach of children and pets. Do not use the medication after its expiration date. Proper storage helps protect the medication from moisture and maintain its effectiveness. Always follow the storage instructions supplied with your specific Foradil product.

Can Foradil Inhaler Interact With Other Medications?

Yes, Foradil Inhaler may interact with certain medications, potentially changing how well your treatment works or increasing your risk of side effects. Foradil contains formoterol fumarate, a long-acting beta2-adrenergic agonist (LABA) that relaxes the muscles surrounding the airways. Although formoterol is inhaled directly into the lungs, some medication can enter your bloodstream and affect other parts of your body. Certain medications may interfere with its bronchodilator effects or increase the risk of cardiovascular complications. For example, beta blockers such as propranolol and metoprolol may reduce the effectiveness of Foradil by blocking beta-adrenergic receptors. Some beta blockers may also cause airway narrowing in people with asthma. Monoamine oxidase inhibitors (MAOIs) and tricyclic antidepressants may increase the cardiovascular effects of formoterol. Combining these medications may raise the risk of a rapid heartbeat, palpitations, or abnormal heart rhythms. Certain diuretics, such as furosemide and hydrochlorothiazide, may increase the risk of low potassium levels when taken alongside Foradil. Low potassium can cause muscle weakness, cramps, and potentially serious heart rhythm problems. Other medications, including theophylline and systemic corticosteroids, may also increase the risk of reduced potassium levels in susceptible individuals. Using Foradil alongside another LABA, such as salmeterol or indacaterol, may lead to excessive beta2-agonist exposure and increase the risk of serious side effects. Do not combine these medications unless your healthcare provider has specifically reviewed your treatment. Some medications that prolong the QT interval may also increase the risk of abnormal heart rhythms when used with formoterol. Not every medication combination causes a significant interaction. However, your healthcare provider should review your complete medication list before starting treatment. Tell your healthcare provider or pharmacist about all prescription medications, over-the-counter drugs, inhalers, vitamins, herbal supplements, and natural health products you use. Do not start, stop, or change another medication without consulting your healthcare provider. If you experience unusual palpitations, severe dizziness, muscle weakness, or worsening breathing symptoms while using Foradil, seek medical advice promptly.
