Table of contents:

TOBI Podhaler capsules are a prescription inhaled medication indicated for the suppressive therapy of chronic pulmonary infection due to Pseudomonas aeruginosa in adults and children aged 6 years and older.

The active ingredient in Tobi Podhaler, tobramycin inhalation powder, is released into the lungs through the Podhaler device, where it targets the bacteria causing the infection. Tobramycin is an antibiotic that works by stopping the growth of bacteria, including Pseudomonas aeruginosa, which is a common bacteria found in the lungs of cystic fibrosis patients.

Use and Dosage

Tobi Podhaler contains 28 mg of Tobramycin Inhalation Powder per capsule. One treatment cycle consists of 28 days on and 28 days off treatment. Each dose of 4 capsules should be taken as close to 12 hours apart as possible; each dose should not be taken less than 6 hours apart. The powder from all 4 capsules must be inhaled to receive the full dose of 112 mg. Inhale 2 times from each capsule to empty it.

Do not swallow the TOBI Podhaler capsules, as the powder in the capsule is for inhalation only. Each capsule should only be removed from the blister immediately before use and should be stored in the blister.

Cautions

  • Allergic reactions: Patients who are allergic to tobramycin or any other aminoglycoside antibiotics should not use Tobi Podhaler.
  • Risk of bronchospasm: Tobi Podhaler may cause bronchospasm (wheezing, shortness of breath, chest tightness). Patients with asthma or chronic obstructive pulmonary disease (COPD) may be at a higher risk of developing bronchospasm.
  • Nephrotoxicity: Tobramycin may cause kidney problems, especially in patients who already have kidney disease or those who receive high doses or prolonged treatment with the drug.
  • Ototoxicity: Tobramycin may cause hearing loss or tinnitus (ringing in the ears), especially in patients who receive high doses or prolonged treatment with the drug.
  • Neuromuscular blockade: Tobramycin may interfere with neuromuscular transmission and cause muscle weakness or respiratory depression, especially in patients who receive high doses or prolonged treatment with the drug.
  • Pregnancy and breastfeeding: Tobi Podhaler should only be used during pregnancy or breastfeeding if the potential benefits justify the potential risks to the fetus or infant.
  • Potential drug interactions: Tobramycin may interact with other medications you are taking, including diuretics, neuromuscular blockers, and other aminoglycoside antibiotics. Your healthcare provider should be aware of all medications you are taking to avoid interactions.
  • Resistance and superinfections: The overuse or misuse of antibiotics such as Tobramycin may lead to the development of bacterial resistance, or the emergence of superinfections caused by bacteria or fungi that are not affected by the drug.
  • Monitoring: Patients using Tobi Podhaler may need to have their lung function, kidney function, and hearing checked regularly to monitor for potential side effects or complications.
  • Inhalation technique: Use the Tobi Podhaler correctly to ensure that the medication reaches the lungs and is effective. Patients should receive proper training on how to use the device and follow the instructions carefully.
  • Storage and handling: Tobi Podhaler capsules should be stored in a cool, dry place and protected from light. Use the capsules within the designated expiry date and discard any unused capsules after 28 days. The device should be handled with clean hands and kept clean to avoid contamination.
