Please leave your name, area code, phone number, and email address during our non-business hours. We will contact you as soon as possible.

Order By Fax or Mail

  2. Complete and sign the form and send to Online Pharmacies Canada with a copy of your original prescription(s). Your prescription will be verified by Online Pharmacies Canada with your physician's office. We also require a copy of one of the following: driver's license, passport, military ID, or birth certificate.
  3. A Patient Care Associate will contact you to confirm your order.
