Pradaxa should be taken exactly as prescribed by your healthcare provider. It is available as an oral capsule that may be taken with or without food. For most conditions, Pradaxa is taken twice daily, with doses spaced approximately 12 hours apart.
Swallow the capsules whole with a full glass of water. Do not crush, chew, break, or open the capsules. Damaging the capsule can cause too much medication to be released at once, which may increase the risk of side effects, including bleeding.
If Pradaxa causes stomach discomfort, taking it with food may help. Speak with your healthcare provider if stomach upset becomes bothersome.
For children who are prescribed Pradaxa, the first dose is usually taken in the morning and the second dose in the evening, approximately 12 hours apart.
If you are taking Pradaxa after hip or knee replacement surgery, follow your healthcare provider’s instructions carefully. Avoid taking antacids within 24 hours after surgery unless directed otherwise, as they may affect how the medication works.
Pradaxa capsules must be stored in their original bottle or blister package to protect them from moisture. Do not transfer the capsules to pill organizers, pill boxes, or medication reminder containers. Always keep the container tightly closed when not in use.
Your dose of Pradaxa will depend on several factors, including your medical condition, kidney function, response to treatment, and any other medications you take.
Recommended Dosage
Non-Valvular Atrial Fibrillation (NVAF)
To help reduce the risk of stroke and systemic embolism in people with non-valvular atrial fibrillation:

- The appropriate dose depends on your kidney function and individual medical needs

To help prevent blood clots after surgery:
- 110 mg by mouth once daily, starting a few hours after surgery as directed by your healthcare provider
- Followed by 220 mg by mouth once daily for the remainder of treatment
Treatment and Prevention of Deep Vein Thrombosis (DVT) and Pulmonary Embolism (PE)
For the treatment of blood clots and to help prevent them from recurring:
- 150 mg by mouth twice daily
- Treatment is usually started after completing 5 to 10 days of injectable blood thinner therapy
Important Dosing Information

- Take doses at about the same times each day.
- Do not skip doses or stop treatment without consulting your healthcare provider.
- Do not crush, chew, or open the capsules.
